Payment options include cash and all major credit cards. We do not accept any form of smartphone payment, such as Apple Pay, Google Pay, or Venmo.
This plan is non-refundable. No refunds or premiums will be issued at any time if a member decides not to utilize his/her dental membership. A member will not receive a membership card. This plan’s effective date will be on file with our office. This program is an in-office discount plan and is not a substitute for a comprehensive dental insurance plan, or in conjunction with another dental plan or offer.
THIS PROGRAM CAN NOT BE USED FOR:
- Services covered under workers’ compensation
- Treatment which, in sole opinion of the treating dentist,
lies outside the realm of their capability - Referrals to specialists
This plan is only covered at Celestial Dental. It can not be used at any other office. The 12-month membership is due in full upon joining. Membership is effective on the day on which payment is received.
Payment is due when services are rendered. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund. Renewal payment is due at the beginning of the same month of original joining each year. Membership rates may be reviewed and adjusted on an annual basis with notification.
STAR MEMBERSHIP IS NOT:
• insurance
• to be combined with insurance
• a qualified health plan under the ACA
• pre-payment for future services
Our in-house plan is a direct payment arrangement for services, products, and care provided annually at Celestial Dental.
